From I-5, (North or South) take the 85th Street exit (Exit #172), which will set you westbound on North 85th Street. Follow 85th west, crossing Aurora Avenue to Dayton Avenue North which is two lights past Aurora Avenue. Quantah is located on the southeast corner of North 85th Street & Dayton Avenue North.   Quantah is in a one-story brick office building, which is shared with law offices. (Law Offices of Michael Tomkins).  Quantah entrance is on the south side of the building and has a green awning over the door with the white letter "Q". Feel free to park in the parking lot provided in any space marked “law office”. Please do not park in the parking lot for Encore Media Group or they may tow your car.
 
Must bring to assessment appointment:
 
- a five-year "complete" driving abstract.  This is obtained from any Washington State Department of Licensing office. (cost $10)
- your case number
- date of offense
- breathalyzer results
- attorney contact information
 
 
Assessment fee is $215. The fee is payable by cash, check or credit card.
